Hidden sources of shellfish, and why iodine has nothing to do with it

Shellfish allergy is one of the most common adult food allergies, and it comes with two problems that are specific to it: a labelling gap that leaves molluscs undeclared on US packaging, and a myth about iodine that has followed patients into hospital waiting rooms for decades. Both are worth understanding properly.

Two groups, and only one of them is always labelled
Shellfish splits into two groups that are not closely related to each other:
- Crustaceans: shrimp, prawns, crab, lobster, crayfish, langoustine. These cause most reactions.
- Molluscs: scallops, oysters, clams, mussels, cockles, winkles, squid, octopus, and land molluscs like snails.
In the US, only crustacean shellfish is a major allergen requiring declaration. Crustaceans must be listed in plain language and the specific variety named. Molluscs are not required to be labelled in the US at this time and may be present unexpectedly. That is a real gap: a US product can contain clam extract without any allergen statement pointing at it.
In the UK and EU the gap does not exist. Both crustaceans and molluscs are on the list of fourteen declarable allergens, and both must be flagged in the ingredient list, for example "mussels (mollusc)" or "prawns (crustaceans)". If you are shopping across those markets, the same product can be labelled very differently.
Can I eat molluscs if crustaceans are the problem?
Often, yes. Many shellfish-allergic people can tolerate molluscs such as scallops, oysters, clams and mussels, because crustaceans are what most reactions involve. But most people allergic to one shellfish do react to others, so allergists generally recommend avoiding all shellfish unless testing says otherwise, and restaurants and fish counters handle both groups on the same surfaces regardless.
Shellfish allergy is not an iodine allergy
This one is worth stating flatly because it still gets repeated by people who should know better.
The allergen in shellfish is tropomyosin, a muscle protein. It is not iodine. Shellfish does contain iodine, but so do dairy, eggs and iodised salt, and the allergy has nothing to do with it. If you have a shellfish allergy you do not need to worry about cross-reactions with iodine, and a shellfish allergy on its own is not a reason to avoid iodine-containing contrast dye for a scan. Tell the radiology team about your allergies, of course, and let them assess the actual risk rather than the folklore.
Shellfish allergy is not fish allergy
Same menu, different animals. Shellfish allergy is different from an allergy to fish, and people allergic to shellfish do not necessarily have to avoid fish, or the other way round. See our fish guide for that side of it.
Where shellfish turns up unexpectedly
- Oyster sauce and fish sauce, and the many dishes built on them
- Fish stock, which frequently contains shellfish, and bouillabaisse
- Seafood flavouring, including crab and clam extract
- Surimi and imitation crab, which are finned fish shaped and flavoured to imitate crab, and often carry genuine crab extract
- Glucosamine supplements, which are commonly made from crustacean shells
- Caesar-style and Asian dressings, curry pastes and stocks
Steam and shared surfaces
Like fish, shellfish can be a problem before it reaches a plate: shellfish protein can be carried in the steam from cooking, and shared fryers, grills and fish counters move it between foods. Guidance is to avoid areas where shellfish is being prepared if you are sensitive; ask your allergist how cautious you personally need to be.
